目的为宁波市开展新生儿甲低筛查工作提供依据,确立宁波市新生儿促甲状腺激素(TSH)参考值范围。方法调查全市5个区和所辖5个县、县级市共33家单位2000年1月至2001年12月在医院出生的61425例新生儿TSH值。调查内容、方法、诊断标准及质控均按新生儿筛查工作标准进行。结果排除测到的先天性甲状腺机能减退症(41例)、单纯高TSH者(25例)和一过性高TSH者(10例)外,对其余61349例新生儿TSH结果作统计学分析。用矩法作正态性检验,TSH属偏态(正偏态)分布将原始数据作为数变换后,仍然呈非正态分布。61349例新生儿TSH最大值为18.8mlU/L,最小值为0.1mlU/L,中值为2.2mlU/L。用百分位数确定正常值范围的界限:单侧上限99%=9.4mlU/L。结论宁波市新生儿甲低筛查TSH(时间荧光分辨法)正常值可定为≤9.4mlU/L。
Objective To provide a basis for carrying out neonatal hypothyroidism screening in Ningbo and to establish the reference value range of neonatal thyrotropin (TSH) in Ningbo. Methods The TSH value of 61,425 newborns born in hospitals from January 2000 to December 2001 in 33 districts of 5 districts and 5 counties and cities under the jurisdiction of the county was investigated. Survey content, methods, diagnostic criteria and quality control according to neonatal screening standards. Results Exclusion of congenital hypothyroidism (41 cases), simple high TSH (25 cases) and transient high TSH (10 cases), the remaining 61349 cases of newborns TSH results for statistical analysis. Using the moment method as the normality test, the TSH is skewed (positively skewed) distribution, the original data as a number of transformation, still non-normal distribution. The maximum TSH of 61349 newborns was 18.8 mlU / L, the minimum was 0.1 mlU / L and the median was 2.2 mlU / L. Use the percentiles to determine the normal range of the limit: unilateral upper limit of 99% = 9.4mlU / L. Conclusion The normal value of neonatal hypothyroidism TSH (time fluorescence method) in Ningbo can be set as ≤9.4mlU / L.
