本文报道2例深部静脉窦系统血栓形成所致的静脉性丘脑梗塞(VTI)。病例1,女,18岁。入院前3天,有项背痛、呕吐、表情淡漠。入院检查:颈强直,中度视乳头水肿,盲点扩大。治疗4天后,左侧第Ⅵ、Ⅶ颅神经麻痹,兼有右侧大脑半球颞额叶内侧病灶性损害。病人有口服避孕药史4周。4个月后,病人完全恢复。发病第4天非增强 CT 和 MRI 均显示深部静脉经路上有高强度信号。双侧横窦也有高信号影。21个月后MRI 示上矢状窦有部分再通的狭窄管腔,两侧横窦闭塞。病例2,女,21岁。突发性头痛,呕吐,两天后出现嗜睡,失语,右侧肢体不全麻痹。继而昏迷。苏醒
This article reports 2 cases of deep venous sinus thrombosis caused by venous thalamic infarction (VTI). Case 1, female, 18 years old. 3 days before admission, a backache, vomiting, apathy. Admission examination: neck stiffness, moderate papilledema, blind spot expansion. After 4 days of treatment, the left Ⅵ, Ⅶ cranial nerve palsy, both the right temporal hemisphere frontal temporal medial lesions lesions. Patients have oral contraceptives for 4 weeks. After 4 months, the patient recovered completely. On the 4th day of onset, non-enhanced CT and MRI showed that there were high-intensity signals in the deep vein. Bilateral transverse sinus also have high signal shadow. After 21 months, MRI showed a partially recanalized stenosis of the sagittal sinus with occlusion of the transverse sinuses on both sides. Case 2, female, 21 years old. Sudden headache, vomiting, two days later lethargy, aphasia, right limb paralysis incomplete. Then coma. wake
