目的:系统评价有关喉癌组织中HIF-1α的表达及临床意义。方法:计算机检索PubMed、EMbase、Web of Science、VIP、CNKI和万方数据库,回顾性收集国内外发表的相关病例-对照研究,检索时间为2000-01-2015-12。将检索文献参照文献质量评价标准进行筛选,并借助RevMan 5.3软件进行Meta分析。结果:共纳入5例患者进行对照研究,包含喉癌组织202例,正常癌旁组织75例。Meta分析结果显示:在HIF-1α表达方面,喉癌组织比正常组织表达高[P<0.01,OR=22.4,95%CI(9.79,51.25)],有淋巴结转移的喉癌组织比无淋巴结转移的喉癌组织表达高[P<0.01,OR=7.10,95%CI(3.10,16.30)],中低分化喉癌组织中的表达较高分化的喉癌组织明显增高[P<0.01,OR=4.40,95%CI(2.28,8.49)];喉癌临床Ⅲ~Ⅳ期较Ⅰ~Ⅱ期HIF-1α表达明显增高[P<0.01,OR=5.89,95%CI(2.44,14.24)]。结论:HIF-1α的表达与喉癌的发生、淋巴结是否发生转移、肿瘤分级以及临床分期密切相关。
Objective: To evaluate the expression of HIF-1α in laryngeal carcinoma and its clinical significance. Methods: PubMed, EMbase, Web of Science, VIP, CNKI and Wanfang database were searched by computer. Relevant case-control studies published at home and abroad were collected retrospectively. The search time was from 2000-01-2015-12. The search literature was screened based on the literature quality evaluation criteria and Meta-analysis was performed with RevMan 5.3 software. Results: A total of 5 patients were enrolled in this study, including 202 cases of laryngeal carcinoma and 75 cases of normal paracancer. Meta-analysis showed that in the expression of HIF-1α, the expression of HIF-1α in laryngeal cancer tissues was higher than that in normal tissues [P <0.01, OR = 22.4,95% CI (9.79,51.25)]. (P <0.01, OR = 7.10, 95% CI (3.10, 16.30)]. The expression of HCC in laryngeal squamous cell carcinoma was significantly higher than that in laryngeal carcinoma [ 4.40, 95% CI (2.28,8.49)]. The expression of HIF-1α in stage Ⅲ-Ⅳ laryngeal cancer was significantly higher than that in stage Ⅰ-Ⅱ [P0.01, OR = 5.89, 95% CI 2.44,14.24]. Conclusion: The expression of HIF-1α is closely related to the occurrence of laryngeal carcinoma, lymph node metastasis, tumor grade and clinical stage.
