为探讨PetCO2在新生儿机械通气中估测PaCO2的可靠性和应用价值,在20例PaO2/FiO2>267kPa,呼吸频率<70次/分的机械通气的新生儿中进行了PetCO2和PaCO2对比研究。PetCO2监测采用气管插管的近端和远端两种方法,并对近端的监测值作了个体化纠正。结果表明在机械通气的新生儿中应用PetCO2监测,无论在气管插管的近端或远端或近端纠正后PetCO2,其与PaCO2相关均良好,r分别为086、092和096。远端监测的方法较近端能更精确地反映PaCO2,但易被分泌物阻塞;近端纠正后的PetCO2值比远端监测更接近PaCO2,能较精确地估计PaCO2,结论为近端PetCO2监测方法简捷、易行、不易被分泌物阻塞,通过个体化纠正后的数据比远端监测更接近PaCO2,便于推广使用
In order to investigate the reliability and application value of PetCO2 to estimate PaCO2 in neonatal mechanical ventilation, PetCO2 and PaCO2 were compared in 20 mechanically ventilated neonates with PaO2 / FiO2> 26.7kPa and respiratory rate <70 beats / min the study. The PetCO2 monitors both proximal and distal tracheal intubation and individually corrects the proximal monitoring values. The results showed that PetCO2 monitoring in mechanically ventilated neonates showed good correlation with PaCO2 regardless of proximal or distal or proximal correction of PetCO2 after tracheal intubation (r = 0.86, 0.92 and 0, respectively) 96. The method of distal monitoring more accurately reflects PaCO2 than the proximal one, but it is easily blocked by secretions.
