目的探讨安氏Ⅱ2错畸形伴单侧颞下颌关节弹响症状患者正畸治疗前后影像学及开闭口髁突运动轨迹的变化。方法选取安氏Ⅱ2伴有单侧关节弹响症状患者20例和个别正常23例,正畸治疗前后拍摄矫正薛氏位片,利用CADIAX记录开闭口运动过程中髁突运动轨迹,并进行定性定量分析。结果58.33%安氏Ⅱ2正畸治疗后髁突位置由后位恢复至中位;治疗前髁突运动轨迹明显垂直化,平滑度及对称性均较差,治疗后则明显改善;治疗前后除(transversal condyle inclination,TCI)及5mm时水平面髁突倾斜度TCI变化不明显,其余研究指标改变均有统计学意义。结论安氏Ⅱ2伴有单侧关节弹响症状患者正畸治疗可使盘突关系恢复正常,且髁突运动也更加协调。
Objective To investigate the changes of condyles movement and imaging before and after orthodontic treatment in Angle Ⅱ 2 malocclusion with unilateral temporomandibular joint snapping symptoms. Methods 20 cases of Angle Ⅱ 2 patients with unilateral joint sore symptoms and 23 cases of normal individual were selected. The orthodontic treatment was performed before and after treatment. The motion track of condyle was recorded by CADIAX and qualitatively Quantitative analysis. Results 58.33% Angle Ⅱ 2 orthodontic treatment after the condyle position from the post-recovery to the median; condylar movement trajectory before treatment was significantly vertical, smoothness and symmetry were poor, significantly improved after treatment; before and after treatment except transversal condyle inclination (TCI) and horizontal condylar inclination TCI did not change obviously at the level of 5mm, and the rest of the research indexes had statistical significance. Conclusions Orthodontic treatment of Angle Ⅱ 2 patients with unilateral stenosis symptoms can restore the relationship between disc herniation and condylar movement more harmoniously.
