目的研究大潮气量机械通气对大鼠肺组织白介素-1β(IL-1β)表达的影响。方法健康SD大鼠24只随机分为3组,对照组(A组)不行机械通气,常规通气组(B组)VT为8ml/kg,大潮气量通气组(C组)VT为40ml/kg。机械通气2h后处死动物,采用考马斯亮蓝染色法测定支气管肺泡灌洗液(BALF)中总蛋白的水平,光镜下行白细胞计数,同时测定肺湿干重比值(W/D)和中性粒细胞髓过氧化物酶(MPO)活性;采用ELISA法检测IL-1β水平,RT-PCR法检测IL-1βmRNA含量。结果通气2h后,与对照组相比,大潮气量通气组BALF中总蛋白、白细胞计数、肺W/D、MPO、IL-1β和IL-1βmRNA表达均显著增加(P<0.05或P<0.01),而B组上述各项指标的变化均无统计学意义(P>0.05)。结论大潮气量机械通气可引起大鼠急性肺损伤,诱导IL-1β表达增加。
Objective To investigate the effect of mechanical ventilation with large tidal volume on the expression of interleukin-1β (IL-1β) in rat lung tissue. Methods Twenty-four healthy SD rats were randomly divided into three groups. The control group (group A) received no mechanical ventilation. The VT of the conventional ventilation group (group B) was 8 ml / kg and that of the large tidal volume ventilation group (group C) was 40 ml / kg. Animals were sacrificed 2h after mechanical ventilation. Total protein in bronchoalveolar lavage fluid (BALF) was determined by Coomassie brilliant blue staining. White blood cell count was counted under light microscope. W / D ratio and neutrophil count Cell myeloperoxidase (MPO) activity was assayed. The level of IL-1β was detected by ELISA and the level of IL-1βmRNA was detected by RT-PCR. Results Compared with the control group, the total protein, leukocyte count, W / D, MPO, IL-1β and IL-1βmRNA in BALF were significantly increased (P <0.05 or P <0.01) , While there was no significant difference between the above indexes in group B (P> 0.05). Conclusion High tidal volume ventilation can induce acute lung injury in rats and induce the expression of IL-1β to increase.
