上颌第二磨牙牙根及根管系统变异较多,结构复杂。通常为近中颊根、远中颊根和腭根3个;少数牙近中或远中颊根与腭根融合,或近、远中颊根融合而成2个牙根;极少数为近、远中颊根和腭根相互融合;发生4个牙根(2个颊根、2个腭根)的情况相当少见(0.28%~1.4%)[1]。本文报道2例上颌第二磨牙为腭侧双根的病例,其中1例近颊根双根管,为4根5根管。1病例资料病例1:患者女,31岁。因“右下后牙咬合不适”于2015-11-02至中国医科大学附属口腔医院就诊。对患者曲面断层片(图1a)和口腔锥形束CT(cone beam computed tomography,CBCT)(图1b)进行分析时偶然发现,曲面断层片右上第二磨牙牙根影像不典型,CBCT右上第二磨牙牙根水平切面观显示为完全分离的2个颊根和2个腭根,且近
