目的:利用16层CT对骨盆多发骨折行三维成像及骨盆血管成像为急诊手术前提供更多的信息。方法:回顾性地分析30例临床急诊疑有骨盆多发骨折并血管损伤来我科行骨性三维成像及盆腔血管成像的患者的影像资料并统计。结果:30例骨盆骨折中骨性三维成像示骨盆皆有不同程度的多发骨折并错位。其中血管成像示25例腹主动脉下段,左右髂总动脉、左右髂内动脉、髂外动脉、及左右股动脉上段显示完好。1例左髂外动脉在同侧的髂骨中段断裂,局部血肿形成;1例骶骨粉粹性骨折致左髂内动脉断裂,局部血肿形成;1例右股骨上段骨折致右股前动脉断裂,局部血肿形成。结论:骨盆多发骨折疑有血管损伤时,应用16层螺旋CT骨性三维成像及骨盆血管成像的检查,应是当前急诊处置该伤病首选的一种简便、快速、高效的诊断方法,可为拟行的手术提供更多的信息。
OBJECTIVE: To provide more information on three-dimensional imaging of pelvic fractures and pelvic angiography before emergency surgery using 16-slice CT. Methods: The imaging data and statistics of 30 cases of clinical emergency diagnosis of multiple pelvic fractures and vascular injury in our department of bony three-dimensional imaging and pelvic vascular imaging were retrospectively analyzed. Results: In 30 cases of pelvic fractures, three-dimensional imaging of the pelvis showed multiple degrees of multiple fractures and dislocation. Angiography showed 25 cases of lower abdominal aorta, left and right common iliac artery, left and right internal iliac artery, external iliac artery, and left and right femoral artery superior segment showed good. One case of the left external iliac artery fractured in the ipsilateral iliac crest and formed a local hematoma. One case of sacral powder fracture caused fracture of the left internal iliac artery and local hematoma. One case suffered fracture of the right femoral artery in the right femoral fracture, Local hematoma formation. CONCLUSIONS: In the case of multiple pelvic fractures suspected vascular injury, the application of 16-slice spiral CT three-dimensional imaging and pelvic vascular imaging examination should be the first choice for emergency treatment of the injury is a simple, rapid and efficient diagnostic method for The proposed surgery provides more information.
