目的:探讨cyclin D1蛋白表达在判断头颈鳞癌患者预后方面的意义。方法 :选取头颈鳞癌患者96例,取肿瘤标本制作组织芯片,应用免疫组化方法检测cyclin D1蛋白的表达情况。蛋白表达与临床病理学参数的关系应用非参数检验(Kruskal-Wallis检验),与预后的关系应用Kaplan-Meier检验和Cox回归多因素生存分析,所有统计学分析均采用SPSS17.0软件包完成。结果:患者平均随访时间为6.2a。Kaplan-Meier生存分析表明,cyclin D1低表达与高表达患者相比预后更好(P=0.009);多因素生存分析表明,cyclin D1蛋白表达是唯一与总生存率密切相关的因子(HR:2.260,95%CI:1.074-4.758;P=0.032)。该蛋白表达与病理分级、临床分期、性别及发病部位等临床病理学参数无关。结论:cyclin D1表达是预测头颈鳞癌患者预后的独立因子,能够作为头颈鳞癌潜在的分子诊断和靶向治疗靶点。
Objective: To investigate the significance of cyclin D1 protein in the prognosis of patients with head and neck squamous cell carcinoma. Methods: Ninety-six patients with squamous cell carcinoma of the head and neck were selected for the production of tissue microarrays. The expression of cyclin D1 protein was detected by immunohistochemistry. The relationship between protein expression and clinicopathological parameters was analyzed by Kruskal-Wallis test. The relationship between the protein expression and clinicopathological parameters was analyzed by Kaplan-Meier test and Cox regression multivariate survival analysis. All the statistical analysis were done by SPSS17.0 software package. Results: The average follow-up time was 6.2 years. Kaplan-Meier survival analysis showed that the low expression of cyclin D1 had a better prognosis than the high expression (P = 0.009). Multivariate survival analysis showed that cyclin D1 protein expression was the only factor closely related to overall survival (HR 2.260 , 95% CI: 1.074-4.758; P = 0.032). The protein expression and pathological grade, clinical stage, gender and pathogenic sites and other clinical pathological parameters. Conclusion: The expression of cyclin D1 is an independent factor for predicting the prognosis of patients with head and neck squamous cell carcinoma and can be used as a potential target for molecular diagnosis and targeted therapy of head and neck squamous cell carcinoma.
