目的:评价牙周病患者种植修复临床疗效。方法:选择回访到的我院2008-2012年因牙周炎拔牙后种植修复的患者78例,169枚种植体,其中前牙54枚,后牙115枚。采用CBCT辅助检查,ISQ值辅助测量,分别行即刻种植修复或者延迟即刻种植修复。CBCT分析术后3、6个月、1年种植体周成骨情况;观察食物嵌塞等并发症的发生情况。结果:1.前牙区2枚植体(初期稳定性25N.cm,行牙槽嵴增厚术)术后两个月发生种植体周围炎,经治疗后恢复正常,修复后一年种植体周成骨情况良好;后牙区1枚种植术后一个月松动脱落,其余166枚种植体术后3、6、12个月检查成骨良好。2.并发症:a、种植体周围炎一例,2枚种植体,经治疗后修复无异常;b、修复后食物嵌塞5例,7枚种植体;c、术后1年牙槽嵴吸收超过2mm的3例,5枚种植体。讨论:1.骨增量技术在牙周病种植修复中广泛应用;2.术前对牙周炎症进行控制;3.正畸治疗介入;4.如何减少术后并发症-完善的术前检查和预判。结论:牙周病患者选择适当的术式行种植修复同样能够取得较为满意的修复效果。
Objective: To evaluate the clinical effect of implant restoration in patients with periodontal disease. Methods: A total of 78 implants and 169 implants were retrospectively retrospectively reviewed from 2008 to 2012 in our hospital, including 54 anterior teeth and 115 posterior teeth. Complications: a, peri-implantitis in one case, two implants, after treatment, no abnormalities were repaired; b, food impaction in 5 cases, 7 implants; c, 1 year after alveolar ridge absorption 3 cases of more than 2mm, 5 implants. Discussions: 1 bone augmentation techniques widely used in periodontal dental implantation; 2 preoperative periodontal disease control; 3 orthodontic treatment intervention; 4 how to reduce postoperative complications - comprehensive preoperative examination... And pre-sentenced. Conclusion: Periodontal disease patients choose the appropriate surgical repair the same line can be obtained more satisfactory repair effect.
