目的探讨超声心肌组织追踪(TTI)技术评价左室节段性室壁运动异常的临床价值。方法采用TTI技术对41例健康人、60例冠心病患者于心尖四腔切面、两腔切面及左室长轴切面检测左室后间隔、侧壁、前壁、下壁、前间隔及后壁收缩期位移,并与冠状动脉造影和二维超声心动图结果比较。结果冠心病患者左室壁运动位移异常节段表现为位移值的降低及其彩色编码显像和曲线波形的改变,TTI诊断冠心病的阳性检出率为87.7%,二维超声心动图诊断冠心病的阳性检出率为68.0%,二者差异有统计学意义(P<0.05)。结论TTI技术能够无创、快速、客观地定量和定性分析左室壁节段性运动异常,明显提高左室壁节段性室壁运动异常的检出率。
Objective To investigate the clinical value of ultrasonic myocardial tissue tracking (TTI) in assessing left ventricular segmental wall motion abnormalities. Methods The TTI technique was used to detect the posterior left ventricular septum, lateral wall, anterior wall, inferior wall, anterior septum and posterior wall in 41 healthy people and 60 patients with coronary heart disease in apical four-chamber view, two-compartment view and left ventricular long axis. Systolic displacement was compared with coronary angiography and two-dimensional echocardiographic results. Results Left ventricular wall motion displacement in patients with coronary artery disease showed abnormal displacement of the displacement value and its color-coded imaging and waveform changes. The positive rate of TTI in diagnosing coronary artery disease was 87.7%. The diagnostic value of two-dimensional echocardiography The positive rate of heart disease was 68.0%, the difference was statistically significant (P <0.05). Conclusion The TTI technique can quantitatively and qualitatively analyze the segmental motility of left ventricular wall in a noninvasive, rapid and objective manner and significantly improve the detection rate of segmental wall motion abnormality in the left ventricular wall.
