目的 探讨非小细胞肺癌cyclinD1基因表达与预后的关系。方法 采用免疫组化Envision二步法对 46例非小细胞肺癌手术标本进行cyc1inD1检测 ,并结合临床随访资料进行分析。 结果 2 5例cyclinD1表达阳性 ,表达率为 5 4.3 % ;其中细胞核阳性表达 6例 ,细胞质阳性表达 19例。临床随访资料显示 ,在cyclinD 1基因表达阳性的 2 5例患者中有 10例术后发生远处转移 ,而cyclinD1表达阴性的 2 1例中 ,仅 1例发生远处转移 ,两者有显著性差异 (P <0 .0 5 )。且随着cyclinD 1阳性表达程度的增高 ,发生转移的概率也增大 ,表达中度阳性 ( ++)的患者其转移发生率明显高于阴性 ( -)者 (P <0 .0 5 ) ,而表达为强阳性 ( +++)者与阴性 ( -)者比较 ,其转移发生率差异非常显著 (P <0 .0 1)。进一步分析还发现 ,在cyclinD1基因表达阳性并发生远处转移的 10例患者中 ,细胞核阳性者 5例 ,其转移发生率高达 83 .3 % ( 5 /6) ,而细胞质阳性的转移发生率只有 2 6.3 % ( 5 /19) ,两者有显著性差异(P <0 .0 5 )。结论 cyclinD1阳性表达与非小细胞肺癌的发生发展有关 ,其表达的强度及部位与远处转移密切相关 ;检测非小细胞肺癌组织中cyclinD1的表达有助于判断预后并对治疗具有一定的指导意义。
Objective To investigate the relationship between cyclinD1 gene expression and prognosis in non-small cell lung cancer. Methods The immunohistochemical Envision two-step method for 46 cases of non-small cell lung cancer specimens cyc1inD1 detection, combined with clinical follow-up data analysis. Results The positive expression rate of cyclinD1 in 25 cases was 5 4.3%. Among them, 6 cases were positive for nuclei and 19 cases were positive for cytoplasm. Clinical follow-up data show that in 25 cases of cyclinD 1 gene expression positive in 10 cases of distant metastasis occurred, and 21 cases of cyclinD1 negative expression in only 1 case of distant metastasis, both were significant Differences (P <0. 05). The positive rate of metastasis increased with the increase of the expression of cyclinD 1. The positive rate of metastasis was significantly higher in patients with positive expression (++) than those with negative (-) (P <0.05) However, there was a significant difference in the incidence of metastasis between those with strong positive (+++) and those with negative (-) (P <0.01). Further analysis also found that among the 10 patients with positive cyclinD1 gene expression and distant metastasis, the positive rate of metastasis was 83.3% (5/6) in 5 cases with positive nuclei, and the positive rate of cytoplasm metastasis was only 2 6.3% (5/19), there was a significant difference between the two (P <0.05). Conclusions The positive expression of cyclinD1 is associated with the occurrence and development of non-small cell lung cancer. The expression intensity and location of cyclinD1 are closely related to the distant metastasis. Detecting the expression of cyclinD1 in non-small cell lung cancer is helpful to predict the prognosis and has some guiding significance for the treatment .
