目的探讨高频超声对四肢神经鞘类肿瘤的诊断价值及声像图特点。方法对10例志愿者健全肢体进行检查,在了解正常四肢神经的形态及走行的基础上,对26例经手术病理证实的四肢神经鞘类肿瘤的声像图进行分析。结果10例正常四肢神经声像图特点为:纵切面呈条索状高回声,横切时呈类圆形的筛网状结构,其周围包绕稍高回声带。18例神经鞘瘤(上肢14例,下肢4例)均为单发,图像特征以椭圆形实性低回声(15/18)为主,边界清晰,有包膜。8例良性纤维瘤(上肢5例,下肢3例)中,6例单发,为实性低回声,内部回声欠均匀,呈梭形为主(6/8),两端与神经相连;2例为多发,呈串珠状。结论四肢神经鞘类肿瘤在声像图上有其特征性表现,超声可为临床提供首选的影像诊断。
Objective To investigate the diagnostic value and sonographic features of high-frequency ultrasound on the nerve sheath tumors of the limbs. Methods Totally 10 healthy volunteers were examined for their healthy limbs. Based on the morphological and pathological features of normal limbs, 26 cases of neurosurgical tumors of the four limbs confirmed by surgery and pathology were analyzed. Results 10 cases of normal limb neurological ultrasonography features: longitudinal section of the cord-like hyperechoic, cross-section was a circular sieve-like structure, surrounded by a slightly higher echo zone. Eighteen cases of schwannoma (14 cases of upper extremity and 4 cases of lower extremity) were single. The features of the images were mainly oval hypoechoic (15/18) with clear boundary and capsule. In 8 cases of benign fibroma (5 cases of upper extremity, 3 cases of lower extremity), 6 cases were solitary, with hypoechoic echoes. The internal echoes were not uniform and were spindle-shaped (6/8), with the nerves connected at both ends. 2 Cases of multiple, beaded. Conclusion The neurosurgical tumors of the extremities have their characteristic manifestation on the sonography. Ultrasound can provide the preferred imaging diagnosis for the clinic.
