目的探讨血清内脂素(visfatin,VF)在妊娠期糖尿病(gestational diabetes mellitus,GDM)中的临床意义及其与胰岛素抵抗(insulin resistance,IR)的关系。方法分析2015年5月至2016年5月在成都市妇女儿童中心医院接受产前检查且住院分娩的GDM孕妇(38例,观察组)和正常孕妇(38例,对照组)的临床资料,比较两组孕妇基础临床指标水平、糖尿病家族史、血清VF、空腹血糖(fasting plasma glucose,FPG)、空腹胰岛素(fasting insulin,FINS)及胰岛素抵抗指数(insulin resistance index,IRI)水平,分析两组孕妇血清VF与相关因素的关系。结果两组孕妇的孕周、身高、孕前体重及体质量指数(body mass index,BMI)、糖尿病家族史差异均无统计学意义(P>0.05);观察组孕妇诊断时的体重及BMI值、血清VF、FPG、FINS及IRI水平均明显高于对照组(P<0.05)。两组孕妇的血清VF水平与其孕前体重及BMI、诊断时体重及BMI、FPG均无相关性,但与FINS及IRI呈显著正相关(P<0.05)。结论血清VF在GDM孕妇的发病中发挥着重要的作用,其与IR有正相关性。
Objective To investigate the clinical significance of serum visfatin (VF) in gestational diabetes mellitus (GDM) and its relationship with insulin resistance (IR). Methods Clinical data of GDM pregnant women (38 cases, observation group) and normal pregnant women (38 cases, control group) receiving prenatal examination and hospital delivery from Chengdu Children’s Hospital from May 2015 to May 2016 were compared. Two groups of pregnant women with basic clinical indicators, family history of diabetes, serum VF, fasting plasma glucose (FPG), fasting insulin (FINS) and insulin resistance index (IRI) Relationship between serum VF and related factors. Results There was no significant difference in gestational age, height, body weight index (BMI) and family history of diabetes between the two groups of pregnant women (P> 0.05). Body weight and BMI, Serum VF, FPG, FINS and IRI levels were significantly higher than the control group (P <0.05). The serum VF levels of the two groups of pregnant women had no significant correlation with body weight and BMI before pregnancy, body weight at diagnosis, BMI and FPG, but positively correlated with FINS and IRI (P <0.05). Conclusion Serum VF plays an important role in the pathogenesis of GDM pregnant women and has a positive correlation with IR.
