目的:观察丙泊酚、小剂量芬太尼和咪达唑仑复合麻醉在消化道内镜检查中的临床应用及麻醉效果观察。方法:选择220例行消化道内镜检查的患者,ASAⅠ~Ⅱ级,术前检查均无严重心脑血管及呼吸系统病史,建立上肢静脉输液通路后依次推注咪达唑仑0.01~0.02mg/kg、芬太尼0.5μg/kg、丙泊酚1~1.5mg/kg,待患者意识、睫毛反射消失后开始检查,检查中若有轻微体动及时追加丙泊酚用量。结果:麻醉后所有患者血压(BP)、心率(HR)和血氧饱和度(SpO2)均有轻微下降,但绝大部分在正常范围,检查均在4~12min顺利完成,检查中呃逆、体动发生率低。结论:丙泊酚和咪达唑仑、芬太尼联合用药有相互协同作用,可减少丙泊酚用量,起效快、不良反应小,可安全地应用于消化道内镜的检查。
Objective: To observe the clinical application of propofol, small dose of fentanyl and midazolam anesthesia in gastrointestinal endoscopy and observation of anesthetic effect. Methods: A total of 220 patients undergoing endoscopic gastrointestinal endoscopy were enrolled. ASA Ⅰ ~ Ⅱ patients with no history of severe cardiovascular and respiratory diseases were enrolled in this study. After establishment of the upper extremity intravenous fluid infusion route, midazolam 0.01-0.02 mg / kg, fentanyl 0.5μg / kg, propofol 1 ~ 1.5mg / kg, until the patient awareness, eyelash reflex began to check, check if there is a slight body movement timely additional propofol dosage. Results: After anesthesia, blood pressure (BP), heart rate (HR) and oxygen saturation (SpO2) decreased slightly in all patients, but most of them were in the normal range. All the tests were completed within 4 ~ 12min. Low rate of movement. Conclusion: Propofol combined with midazolam and fentanyl have synergistic effect, which can reduce the dosage of propofol, have fast onset of action, little adverse reaction, and can be safely applied to the examination of gastrointestinal endoscopy.
