我院急诊科重症监护病房(ICU)自1991年7月启用以来,对9例危重病人进行了有创性血流动力学监 测,对指导临床诊断、用药、估计疾病预后、提高危重症抢救成功率具有重要的临床意义。 临床资料 一、9例均为ICU监护病者,男6例,女3例;除1例为23岁的散发性脑炎患者外,其余8例均为56~78岁的心或脑血管病患者。 二、本组所用的是7.5FSwan-Ganz漂浮导管,西门子公司生产的SIEMENS730型或960型床边心电监护仪测定血流动力学各项参数。采取左锁骨下静脉穿刺术,通过压力监测判定导管位置。压力单位为kPa,心排出量(CO)单位为L/min。 三、本组置管监测成功8例,失败1例,为误穿入颈总静脉上段。9例中治愈4例,好转1例,死亡4例。
