下消化道大出血是指十二指肠悬韧带以下肠道在短期内出现大量鲜血或暗红稀便,且伴血流动力学变化,如脉速、低血压甚至休克等现象.由于出血病因众多,临床诊断不易,紧急结肠镜检查不但可以明确诊断,且还可同时对部分病灶进行治疗.本院1985～1995年曾对36例下消化道大出血病例进行紧急结肠镜检查,在输血补液支持疗法下待生命体征稳定后,清水灌肠一次或不灌肠,即行检查,循腔进镜,至盲肠末端发现出血灶即停止检查,发现结肠息肉等病灶,行内镜下电凝、电切治疗或安排剖腹手术.
