目的 探讨核的形态参数在膀胱移行细胞肿瘤诊断与鉴别诊断中的作用,为计算机辅助病理诊断积累资料。方法 采用图像分析系统对膀胱慢性炎、内翻性乳头状瘤、移行细胞乳头状瘤(以上称非癌性病变)、移行细胞癌(TCC)Ⅰ级、Ⅱ级、Ⅲ级各5 例及尸检正常移行上皮2 例的核形态参数,(包括核面积、核周长、核形状因子、等效核直径、核质比和核黑度)进行测量。结果 核面积、核周长、等效核直径和核质比四项指标,良恶之间比较均有高度显著性差异,且TCCⅢ> TCCⅡ> TCCⅠ> 非癌性病变。结论 这些参数在良恶鉴别和移行细胞癌的分级中有一定价值;移行细胞癌恶性程序的增高主要表现为细胞核的增大和大小悬殊;图像分析为移行细胞肿瘤的数学病理学研究提供了一定资料。
Objective To investigate the role of nuclear morphological parameters in the diagnosis and differential diagnosis of bladder transitional cell tumors and to accumulate data for the diagnosis of computer-aided pathology. Methods The image analysis system was used in the study of five cases of chronic bladder inflammation, inverted papilloma, transitional cell papilloma (referred to as non-cancerous lesions), grade Ⅰ, Ⅱ and Ⅲ of transitional cell carcinoma (TCC) Nuclear morphological parameters, including nuclear area, perinuclear length, nuclear shape factor, equivalent nuclear diameter, nuclear mass ratio and nuclear blackness, were measured in 2 normal epithelial cells. Results Nuclear area, perinuclear circumference, equivalent nuclear diameter and nuclear mass ratio of four indicators, the difference between the two were highly significant difference, and TCC Ⅲ> TCC Ⅱ> TCC Ⅰ> non-cancerous lesions. Conclusion These parameters are of great value in the differential diagnosis of benign and malignant tumors and in the classification of transitional cell carcinoma. The increase of malignant transitional cell carcinomas is mainly due to the increase and size disparity of nuclei. The image analysis provides certain information for the mathematical pathological study of transitional cell tumors .
