目的分析脑卒中急性期心电图异常改变的情况,为临床治疗和预后估计提供依据。方法采用前瞻性队列研究方法,取自脑卒中患者急性期间心电图记录常规及全导联图形。采用logistic多因素逐步回归分析心电图异常改变与脑卒中急性期病变部位、性质、范围、意识障碍程度及预后等多方面关系。结果共纳入脑卒中急性期患者400例中,出现心电图异常发生率70%,其中出血性卒中占88.9%,缺血性卒中占54.5%,揭示心电图异常发生率与病变部位和意识障碍程度密切相关,与病变性质,范围无关,脑卒中急性期死亡者92.9%出现心电图异常改变。结论脑卒中急性期进行心电图动态监测对于判断脑血管病变的严重程度、治疗方法、预后估计均有着重要临床参考价值。
Objective To analyze the abnormal changes of electrocardiogram in the acute stage of stroke and provide the basis for clinical treatment and prognosis estimation. Methods A prospective cohort study was performed using electrocardiographic recording of conventional and all-lead patterns during the acute period of stroke. Multi-step logistic regression analysis was used to analyze the relationship between abnormal changes of electrocardiogram and the location, nature, extent, disturbance of consciousness and prognosis in the acute stage of stroke. Results A total of 400 acute stroke patients were included. The incidence of electrocardiogram abnormalities was 70%. Among them, hemorrhagic stroke accounted for 88.9% and ischemic stroke accounted for 54.5%. The incidence of electrocardiogram abnormalities was closely related to the lesion location and degree of disturbance of consciousness , With the nature of the disease, the scope has nothing to do, 92.9% of the acute stroke death occurred abnormal ECG changes. Conclusions The dynamic monitoring of ECG during the acute phase of stroke has important clinical value for judging the severity of cerebrovascular disease, treatment and prognosis.
