目的探讨贝尼地平对原发性高血压患者24 h动态血压疗效及对靶器官的影响。方法连续选取2013年6月1日-12月31日在承德医学院附属医院心内科门诊及住院部首次确诊为原发性高血压的患者共197例,口服贝尼地平(4 mg,1次)连续治疗3个月。治疗前后分别进行24 h动态血压检测,并记录TG、TC、HDL-C、LDL-C、肌酐、尿酸、尿素氮、β微球蛋白、心电图、心脏超声等相关指标。计量资料组内比较采用配对t检验。结果治疗3个月后,本组患者24 h血压分布的形态变化、24 h平均血压、白昼平均血压、夜间平均血压、24 h收缩压变异率、24 h舒张压变异率均低于治疗前,差异具有统计学意义(均P<0.05);HDL-C、LDL-C、TG、TCH水平低于治疗前,但差异无统计学意义(均P>0.05);治疗后B微球蛋白、尿素氮、肌酐、尿酸水平均低于治疗前,差异具有统计学意义(均P<0.05);治疗后心电图ST段压低例数较前减少(P<0.05);治疗前后E/A值差异有统计学意义(P<0.05)。结论贝尼地平能24 h长期而缓慢的降血压,效果好,且可同时减少蛋白尿、保护肾功能,无明确降脂作用;贝尼地平还具有改善左室功能作用,是临床应用前景广泛的一类降压药物。
Objective To investigate the effect of benidipine on ambulatory blood pressure and target organ in patients with essential hypertension at 24 hours. METHODS: A total of 197 consecutive patients with primary hypertension diagnosed as primary hypertension at outpatient department and inpatient department of Chengde Medical College Hospital from June 1, 2013 to December 31, 2013 were enrolled in this study. Benidipine (4 mg, once ) For 3 months. Ambulatory blood pressure was measured 24 h before and after treatment, and TG, TC, HDL-C, LDL-C, creatinine, uric acid, urea nitrogen, β microglobulin, electrocardiogram and echocardiography were recorded. Measurement data within the group using paired t test. Results The changes of 24 h blood pressure distribution, 24 h mean blood pressure, mean daytime blood pressure, mean nocturnal blood pressure, 24 h systolic blood pressure variability, 24 h diastolic blood pressure variability were lower than those before treatment, (P <0.05). The levels of HDL-C, LDL-C, TG and TCH were lower than those before treatment, but the difference was not statistically significant (all P> 0.05) (P <0.05). After treatment, the number of ST segment depression in ECG decreased compared with that before treatment (P <0.05). There was statistical difference in E / A value before and after treatment Significance (P <0.05). Conclusion Benidipine can prolong and slow blood pressure lowering for 24 h with good effect and can reduce proteinuria and renal function at the same time without definite lipid-lowering effect. Benidipine also has the effect of improving left ventricular function and has wide application prospects A class of antihypertensive drugs.
