目的:建立河北省儿童促甲状腺激素(TSH)及甲状腺素(T3、T4、FT3、FT4)参考区间,并应用于临床筛查儿童甲状腺疾病。方法:选择黄骅市、保定市、张家口市健康儿童,测定血清中TSH、T3、T4、FT3、FT4,建立参考区间。分别将新建立的参考区间与试剂盒提供的参考区间作为实验室诊断标准,分析诊断三个地区800例儿童的甲状腺疾病发病情况。结果:建立的河北省儿童促甲状腺激素及甲状腺激素参考区间是:TSH 0.50~6.88μIU/ml,T30.52~2.36 ng/ml,T445.28~127.42 ng/ml,FT32.30~10.29 pmol/L、FT47.99~24.88 pmol/L。800例儿童采用该参考区间作为诊断标准,共检出45例异常结果;采用试剂盒提供的参考区间作为诊断标准,共检出120例异常。结论:该参考区间为河北省儿童促甲状腺激素提供了基础性数据,有利于儿童亚临床甲状腺减退的监测工作。
Objective: To establish a reference interval for thyrotropin (TSH) and thyroxine (T3, T4, FT3, FT4) in children with thyroid disease in Hebei Province. Methods: Healthy children in Huanghua City, Baoding City and Zhangjiakou City were selected for determination of serum TSH, T3, T4, FT3 and FT4, and a reference interval was established. The newly established reference intervals and reference ranges provided by the kit were used as laboratory diagnostic criteria to analyze the incidence of thyroid diseases in 800 children in three regions. Results: The reference intervals for thyrotropin and thyroid hormone in Hebei children were: TSH 0.50-6.88μIU / ml, T30.52-2.26ng / ml, T445.28-127.42ng / ml, FT32.30-10.29pmol / L, FT47.99 ~ 24.88 pmol / L. A total of 800 children with this reference interval were used as the diagnostic criteria, and 45 abnormal results were detected. Using the reference interval provided by the kit as the diagnostic criteria, a total of 120 abnormalities were detected. Conclusion: This reference interval provides basic data for thyrotropin of children in Hebei province, which is beneficial to the monitoring of subclinical hypothyroidism in children.
