目的利用PDCA循环改进方法,最大程度地通过技术手段得到优质的单光子发射计算机体层摄影术(SPECT)骨显像图像。方法首先对2012年上半年SPECT全身骨显像图像质量进行回顾性分析和总结,提出整改措施和意见并落实到个人。在2012下半年的每月月末,应用图像存储与传输系统,对骨扫描图像质量进行盲法评比和讨论,评比出骨显像病例数量最多、注射效果最好、图像质量最好的3个项目。对质量欠佳的图像,从患者摆位、药物注射、图像处理的技术操作细节分析原因并形成改进措施。结果利用PDCA循环改进操作技术,2012年下半年7 186幅全身骨扫描图片,优质片达96.51%(6 935/7 186),渗漏率减至8.76%(630/7 186),数量和质量相比上半年得到较大提高。并且从分散、孤立的个人经验中总结出严谨、细致、可操作性强的检查流程和规范,对长期有效提高SPECT全身骨显像图像质量有很好的促进作用。结论在SPECT全身骨显像中应用PDCA方法,促进了医患沟通和理解,检查准备和检查操作做到了既现范化又个性化,提高了扫描图像质量。
Objective To improve the quality of single photon emission computed tomography (SPECT) bone imaging images by using PDCA method. Methods Firstly, the image quality of SPECT bone scintigraphy was retrospectively analyzed and summarized in the first half of 2012, and the corrective measures and opinions were put forward and implemented to individuals. At the end of each month of the second half of 2012, the system of image storage and transmission was used to evaluate and discuss the bone scan image quality blindly. The three projects with the highest number of bone imaging, the best injection and the best image quality were evaluated . The poor quality of the images, from the patient positioning, drug injection, image processing technical analysis of the reasons for the details and the formation of improvements. Results Using PDCA cycle to improve operation technique, in the second half of 2012, a total number of 7 186 whole-body bone scans were obtained, with 96.51% (6 935/7 186) for high-quality tablets and 8.76% (630/7 186) for leakage rate. The quantity and quality Compared to the first half has been greatly improved. And from the scattered, isolated personal experience summed up the rigorous, detailed and operational inspection procedures and norms, the long-term effective to improve the quality of SPECT bone imaging has a good role in promoting. Conclusion The application of PDCA method in SPECT whole body bone imaging has promoted the communication and understanding between doctors and patients. The preparation and inspection operations of inspection are both standardized and personalized, and the quality of scanned images is improved.
