目的通过与18F脱氧葡萄糖(FDG)PET显像对比,探讨201Tl、123Iβ甲基碘苯脂十五烷酸(BMIPP)心肌SPECT显像在评价存活心肌中的临床价值。方法31例急性心肌梗塞患者,将其左心室分成9个节段,采用4级目测评分法测定并比较201Tl及123IBMIPP的聚集(摄取正常=0,严重减少=3)。在201TlSPECT所示血流低下部位,定性及定量测定18FFDG摄取,同时将201Tl、123IBMIPPSPECT及18FFDGPET所见与室壁运动作对比分析。结果血流灌注低下的112个节段中,201Tl、123IBMIPP匹配的节段为429%(48个),不匹配的节段为571%(64个);且201Tl、123IBMIPP不匹配节段的922%和匹配节段的521%(201Tl、123IBMIPP均为严重缺损者占333%),18FFDGPET显示有心肌活力。在左室无运动或者反向运动的节段中,18FFDGPET判断有活力者为656%,201Tl、123IBMIPPSPECT判断有活力者为500%。结论联合201Tl、123IBMIPP心肌SPECT判断存活心肌是有用的,但与?
Objective To investigate the clinical value of 201Tl, 123Iβ methyl iodide phenylpentadecanoic acid (BMIPP) myocardial SPECT imaging in the evaluation of viable myocardium by contrast with 18Fdeoxyglucose (FDG) PET imaging. Methods Thirty-one patients with acute myocardial infarction were divided into 9 segments by left ventricle. The visual acuity of grade 4 was measured and the aggregation of 201T1 and 123IBMIPP were compared (uptake normal = 0, severe reduction = 3). 201TlSPECT showed low blood flow sites, qualitative and quantitative determination of 18F FDG uptake, while the 201Tl, 123I MBIPPSPECT and 18F FDG PET findings and wall motion for comparative analysis. Results Among the 112 segments with low perfusion, the matched segments of 201T1 and 123IBMIPP were 429% (48) and the unmatched segments were 571% (64), and those of 201T1 and 123I 922% of MIPPIPP mismatch segments and 521% of matched segments (33.3% of 201Tl and 123IBMIPP were all severely impaired), 18FFGPGET showed myocardial viability.
