[目的]通过研究胃癌组织中Hp感染与HIF-2α、Oct-4蛋白表达的相关性,初步探讨Hp感染在胃癌发生发展中的干细胞机制及对化疗抵抗的分子机制。[方法 ]采用快速尿素酶试验和免疫组织化学SP法分别检测80例行胃癌手术切除的组织标本(研究组)和60例慢性胃炎患者经胃镜下取组织活检标本(对照组)中的Hp,采用免疫组织化学SP法检测Hp阳性和Hp阴性的研究组组织中HIF-2α、Oct-4蛋白的表达。[结果]研究组Hp阳性率明显高于对照组(χ2=5.865,P=0.012)。研究组中T3+T4Hp阳性率明显高于T1+T2(χ2=9.113,P=0.003),有淋巴结转移Hp阳性率明显高于无淋巴结转移(χ2=6.236,P=0.013)。研究组中Hp阳性的HIF-2α蛋白阳性表达率明显高于Hp阴性,Hp感染与HIF-2α蛋白表达呈正相关(r=0.311,P=0.007);Hp阳性的Oct-4阳性表达率明显高于Hp阴性,Hp感染与Oct-4蛋白表达呈正相关(r=0.324,P=0.005),HIF-2α蛋白表达与Oct-4蛋白表达呈正相关(r=0.560,P=0.000)。[结论]Hp可能通过激活HIF-2α-Oct-4通路来促进胃癌组织中HIF-2α和Oct-4的表达,进而诱导胃癌细胞的干细胞化及化疗抵抗。
[Objective] To investigate the relationship between Hp infection and HIF-2α, Oct-4 protein expression in gastric cancer and to explore the molecular mechanism of Hp infection in the development and progression of gastric cancer stem cells. [Method] Rapid urease test and immunohistochemical SP method were used to detect 80 cases of gastric cancer resected tissue specimens (study group) and 60 cases of chronic gastritis patients underwent endoscopic biopsy specimens (control group) Hp, Immunohistochemical SP method was used to detect the expression of HIF-2α and Oct-4 protein in Hp-positive and Hp-negative study groups. [Results] The positive rate of Hp in study group was significantly higher than that in control group (χ2 = 5.865, P = 0.012). The positive rate of T3 + T4Hp in study group was significantly higher than T1 + T2 (χ2 = 9.113, P = 0.003). The positive rate of Hp in lymph node metastasis was significantly higher than that in non-lymph node metastasis (χ2 = 6.236, P = 0.013). Hp-positive HIF-2α protein expression in the study group was significantly higher than Hp-negative, Hp infection was positively correlated with HIF-2α protein expression (r = 0.311, P = 0.007); Hp positive Oct-4 positive expression rate was significantly higher There was a positive correlation between Hp infection and Oct-4 protein expression (P = 0.000). The positive correlation was found between Hp-2α protein expression and Oct-4 protein expression (r = 0.560, P = 0.000). [Conclusion] Hp may promote the expression of HIF-2α and Oct-4 in gastric cancer tissue by activating HIF-2α-Oct-4 pathway and induce the induction of stem cell and chemoresistance in gastric cancer cells.
