目的 :探讨ras基因突变与眼睑恶性肿瘤发生的关系及临床病理学意义。方法 :收集 98例病理证实的眼睑恶性肿瘤组织 ,其中基底细胞癌 (basalcellcarcinoma ,BCC) 4 0例、鳞状细胞癌 (squamouscellcarcino ma ,SCC) 35例、眼睑皮脂腺癌 2 3例。采用免疫组化SABC法检测p2 1ras蛋白的表达。结果 :眼睑正常及轻度不典型增生上皮中未见p2 1ras表达 ;而中、重度不典型增生 ,原位癌 ,浸润癌p2 1ras阳性表达率逐渐增高 ,随着肿瘤分化程度的降低 ,p2 1ras阳性表达率及阳性表达强度逐渐增高 ;SCC中p2 1ras阳性表达强度与组织分化程度之间呈显著负相关 ,且差异有显著性 (P <0 .0 5 )。p2 1ras蛋白表达与眼睑恶性肿瘤术后复发无关 ,但与淋巴转移有关 (P <0 .0 5 )。结论 :ras基因突变与眼睑恶性肿瘤的发生密切相关 ,出现于SCC发生的早期 ,可能与眼睑恶性肿瘤的恶性进展及恶性程度有关 ;p2 1ras蛋白的高表达者较无表达和低表达者易发生淋巴结转移
Objective: To investigate the relationship between ras gene mutation and eyelid malignancy and its clinicopathological significance. Methods: Ninety-eight cases of pathologically confirmed eyelid malignant tumors were collected, including 40 cases of basal cell carcinoma (BCC), 35 cases of squamous cell carcinoma (SCC) and 23 cases of eyelid sebaceous carcinoma. Immunohistochemical SABC method was used to detect p2 1ras protein expression. Results: No expression of p21ras was found in the normal and mild atypical hyperplasia epithelium of the eyelid. However, the positive rate of p21ras expression in moderate and severe atypical hyperplasia, carcinoma in situ and invasive carcinoma was gradually increased. With the decrease of tumor differentiation, p21ras The positive expression rate of p21ras in SCC was significantly negatively correlated with the degree of tissue differentiation (P <0.05). The expression of p1ras protein had no correlation with postoperative recurrence of eyelid malignant tumor, but it was associated with lymphatic metastasis (P <0.05). Conclusion: The mutation of ras gene is closely related to the occurrence of malignant eyelid tumors and occurs in the early stage of SCC, which may be related to the malignant progression and malignancy of eyelid malignancies.
