目的 :探讨肺癌组织中缺氧诱导因子-1α(hypoxia-inducible factor 1α,HIF-1α)、环氧化物酶-2(cyclooxygenase 2,COX-2)的表达及其与病灶部位微血管密度(microvessel density,MVD)、肺癌临床分期和病理分型的相互关系。方法:免疫组化法检测48例肺癌组织中HIF-1α、COX-2的表达,用CD34单克隆抗体标记血管内皮细胞并计数MVD。结果:48例肺癌组织中HIF-1α的阳性表达率为87.50%(42/48),HIF-1α的表达与组织病理分型无关(P>0.05)。HIF-1α在早期(Ⅰb~Ⅲa期)肺癌组的阳性表达率为78.95%(15/19),在晚期(Ⅲb~Ⅳ期)肺癌组为93.10%(27/29),两组差异有统计学意义(P<0.05)。48例肺癌组织中COX-2阳性表达率为77.08%(37/48),COX-2的表达与组织病理分型无关(P>0.05)。早期肺癌组COX-2的阳性表达率为57.89%(11/19),晚期为89.66%(26/29),两组差异有统计学意义(P<0.05)。肺癌组织中HIF-1α、COX-2的表达呈正相关(P<0.05)。随着HIF-1α或COX-2表达强度的增加,病灶部位MVD值增高(P<0.05)。MVD在HIF-1α、COX-2均为阳性表达的组织中的值显著高于其在两者均为阴性表达的组织中的值或其中任一因素为阳性表达的组织中的值(P<0.05)。结论:HIF-1α和COX-2在肺癌及其血管形成过程中起着重要作用,并且两者间可能存在一定的协同关系。
Objective: To investigate the expression of hypoxia-inducible factor 1α (HIF-1α) and cyclooxygenase-2 (COX-2) in lung cancer and its relationship with the microvessel density , MVD), clinical stage and pathological classification of lung cancer. Methods: The expressions of HIF-1α and COX-2 in 48 cases of lung cancer were detected by immunohistochemistry. The vascular endothelial cells were labeled with CD34 monoclonal antibody and MVD was counted. Results: The positive expression rate of HIF-1α in 48 cases of lung cancer was 87.50% (42/48). The expression of HIF-1α was not related to the histopathological classification (P> 0.05). The positive expression rate of HIF-1α in lung cancer group (78.9%) was significantly higher than that in the early stage (stage Ⅰb-Ⅲa), and 93.10% (27/29) in the advanced stage Ⅲb-Ⅳ lung cancer group Significance (P <0.05). The positive expression rate of COX-2 in 48 cases of lung cancer was 77.08% (37/48), the expression of COX-2 had no correlation with histopathological type (P> 0.05). The positive expression rate of COX-2 in early lung cancer group was 57.89% (11/19) and in late stage was 89.66% (26/29), the difference was statistically significant (P <0.05). The expression of HIF-1α and COX-2 in lung cancer tissues was positively correlated (P <0.05). MVD increased with the increase of HIF-1α or COX-2 expression (P <0.05). MVD was significantly higher in HIF-1α, COX-2 positive tissues than in both negative tissues or in tissues where either factor was positive (P < 0.05). Conclusion: HIF-1α and COX-2 play an important role in the development of lung cancer and its blood vessels, and there may be some synergistic relationship between them.
