目的:研究大承气汤改善阳明腑实喘满证中肺通气功能和肺组织病变的机制。方法:将家兔随机分为对照组(A)、治疗组(B)和大承气汤组(C)。从兔耳缘静脉注入油酸建立家兔实验性肺水肿的动物模型。大承气汤以胃管插入进行治疗,观察动物一般情况、血气分析及形态学改变等指标。结果:油酸可以致家兔实验性肺水肿,以肺水肿、肺出血、毛细血管微血栓和肺泡内透明膜形成为主要标志,同时伴多脏器损伤。大承气汤具有改善肺水肿、促进肺泡上皮增生特别是Ⅱ型上皮增生、保护多脏器功能,促进损伤修复的作用。结论:大承气汤的治疗作用可能与其促进水肿液的吸收、促进肺泡上皮增生特别是Ⅱ型上皮增生与修复、改善肺泡/通气血流比例、保护多脏器以及改善内环境等多种作用有关。
OBJECTIVE: To study the mechanism of Dachengqi Decoction in improving pulmonary ventilation function and pathological changes of lung tissue in Yangming cirrhosis. Methods: Rabbits were randomly divided into control group (A), treatment group (B) and Dachengqi Decoction group (C). An animal model of experimental pulmonary edema in rabbits was established by injecting oleic acid from rabbit ear veins. Dachengqi Decoction was treated with gastric tube insertion. Animals were observed for general conditions, blood gas analysis, and morphological changes. Results: Oleic acid can cause experimental pulmonary edema in rabbits. Pulmonary edema, pulmonary hemorrhage, micro-thrombosis in capillaries, and hyaline membrane formation in the alveoli are the main signs, with multiple organ injuries. Dachengqi Decoction can improve pulmonary edema, promote alveolar epithelial hyperplasia, especially type II epithelial hyperplasia, protect multiple organ functions, and promote injury repair. Conclusion: The therapeutic effect of Dachengqi Decoction may be related to the promotion of absorption of edema fluid, promotion of alveolar epithelial hyperplasia, especially type II epithelial hyperplasia and repair, improvement of alveolar/ventilation blood flow ratio, protection of multiple organs and improvement of internal environment, etc. related.
