目的分析酶联免疫吸附试验(ELISA)检测流行性出血热(EHF)病毒抗体的诊断价值。方法 162例临床初步诊断中怀疑EHF的患者,检测其免疫球蛋白(Ig)M、IgG抗体水平。结果 ELISA法检测IgM抗体的灵敏度达83.8%(31/37),特异性达99.2%(124/125),阳性预测值达96.9%(31/32),阴性预测值达95.4%(124/130),诊断有效率达95.7%(155/162)。ELISA法检测IgG抗体的灵敏度达70.3%(26/37),特异性达98.4%(123/125),阳性预测值达92.9%(26/28),阴性预测值达91.8%(123/134),诊断有效率达92.0%(149/162)。结论 ELISA对EHF的IgM抗体和IgG抗体具有操作简单、检出率高的特点,适合在基层医院推广应用。同时IgM由于更好的检测准确率,适合应用于首选诊断方法。
Objective To analyze the diagnostic value of enzyme-linked immunosorbent assay (ELISA) in the detection of antibodies to epidemic hemorrhagic fever (EHF) virus. Methods A total of 162 patients with suspected EHF during initial diagnosis were enrolled in this study. The levels of IgM M and IgG were detected. Results The sensitivity and specificity of ELISA were 83.8% (31/37), 99.2% (124/125), 96.9% (31/32), and the negative predictive value was 95.4% (124/130) ), The diagnostic efficiency was 95.7% (155/162). The sensitivity of ELISA was 70.3% (26/37), the specificity was 98.4% (123/125), the positive predictive value was 92.9% (26/28) and the negative predictive value was 91.8% (123/134) , The diagnostic efficiency of 92.0% (149/162). Conclusion The ELISA for EHF IgM antibodies and IgG antibodies with simple operation, high detection rate, suitable for promotion and application in primary hospitals. At the same time IgM due to better detection accuracy, suitable for the preferred diagnostic method.
