目的探讨系统性红斑狼疮并发白内障患者手术治疗的临床效果。方法回顾性地总结分析2008年1月—2011年10月在杭州市红十字会医院手术的46例(92眼)系统性红斑狼疮并发白内障患者的临床资料。所有患者行白内障超声乳化联合人工晶体植入术,术后定期随访观察,记录患者术眼视力、眼压和眼底情况,分析术后视力、术后并发症等。应用配对t检验,对术前、后视力进行统计学分析。术后随访时间为3~18个月,平均10个月。结果患者术前患眼最佳矫正视力平均为LogMAR 1.37±0.52,术后1周平均视力为LogMAR 0.39±0.46,术后末次随访平均视力为LogMAR 0.36±0.41,其中最佳矫正视力>0.8者68眼,>0.3者22眼,<0.3者2眼,术后较术前视力有明显提高,差异有统计学意义(t=11.82,P<0.05,t=13.70,P<0.05)。6眼术后发生葡萄膜炎,3眼术后出现后发性白内障,经治疗后视力好转。2眼术后检查发现有明显的眼底病变,视力较差。结论系统性红斑狼疮并发白内障患者行白内障手术疗效肯定,安全性高,术后定期随访可以及时发现和处理并发症如葡萄膜炎、后发性白内障、眼底病变等,有利于维持良好视力,提高患者的生活质量。
Objective To investigate the clinical effects of surgical treatment of patients with systemic lupus erythematosus complicated with cataract. Methods The clinical data of 46 patients (92 eyes) with systemic lupus erythematosus complicated with cataract who underwent surgery at Hangzhou Red Cross Hospital from January 2008 to October 2011 were retrospectively analyzed. All patients underwent cataract phacoemulsification and intraocular lens implantation. The patients were followed up regularly. The visual acuity, intraocular pressure and ocular fundus were recorded. The postoperative visual acuity and postoperative complications were analyzed. Paired t-test, preoperative and postoperative visual acuity were analyzed. Postoperative follow-up time was 3 to 18 months, an average of 10 months. Results The best corrected visual acuity of the affected eyes was LogMAR 1.37 ± 0.52 before surgery and LogMAR 0.39 ± 0.46 at 1 week after surgery. The average follow-up time was LogMAR 0.36 ± 0.41, and the best corrected visual acuity was> 0.8 68 There were 22 eyes (> 0.3) and 2 eyes (0.3 eyes). The postoperative visual acuity was significantly higher than that before operation (t = 11.82, P <0.05, t = 13.70, P <0.05). Ureosynovitis occurred in 6 eyes and postoperative cataract in 3 eyes. Visual acuity improved after treatment. 2 postoperative examination found significant retinopathy, poor vision. Conclusions Cataract patients with systemic lupus erythematosus complicated by cataract surgery have a certain effect and high safety. Regular follow-up after operation can detect and treat complications such as uveitis, posterior capsular disease and ocular fundus in time, which is good for maintaining good vision and improving Quality of life of patients.
