小组背景浙江省平湖市当湖街道三港新城是新建的动拆迁小区,社区内普遍为空巢老人、独居老人、身患疾病老人,他们在物质方面比较富足,但由于搬迁,与人接触、交流沟通的机会减少,身体机能下降或生活兴趣下降,导致其对人生认同感下降的问题日益突出,精神上容易产生孤独和失落感。针对此现象,平湖市心怡社工服务中心(以下简称心怡)结合社区居家养老服务项目,将三港新城社区60岁左右、具有上述同类问题
