目的探讨煤工尘肺(CWP)患者肺泡巨噬细胞(AM)凋亡情况、凋亡蛋白的表达情况及其影响因素。方法选取24例接触煤尘或煤矽尘的观察对象和44例CWP患者为研究对象,收集其大容量肺灌洗回收液,分离纯化AM,离体培养24 h,苏木素-伊红(HE)染色,光学显微镜下观察AM的形态特征,用Western blot方法检测死亡受体(DR)5、Caspase-3和Caspase-8的相对水平。结果凋亡的AM染色质浓缩、边缘化、分割成块状;CWP患者AM的凋亡指数高于观察对象(P<0.05),且贰期患者高于壹期患者(P<0.05);观察对象DR5、Caspase-3和Caspase-8的表达量均低于CWP患者(P<0.05);接尘工龄≥20年组DR5、Caspase-3和Caspase-8的表达水平高于<20年组(P<0.05);发病工龄<10年组DR5和Caspase-3的表达水平高于≥10年组(P<0.05)。结论 DR5、Caspase-8和Caspase-3在CWP的发生发展中可能发挥着重要的作用,且凋亡蛋白的表达水平与接尘工龄、发病工龄有关。
Objective To investigate the apoptosis of alveolar macrophages (AM) and the expression of apoptosis proteins in coal worker’s pneumoconiosis (CWP) patients and its influencing factors. Methods Totally 24 subjects exposed to coal dust or coal silica dust and 44 CWP patients were selected as research objects. The large volume of lung lavage solution was collected, and AM was isolated and purified. Hematoxylin and eosin (HE) Morphological characteristics of AM were observed under optical microscope. The relative levels of death receptor (DR) 5, Caspase-3 and Caspase-8 were detected by Western blot. Results Apoptotic AMs were concentrated, marginalized and divided into blocks. The apoptotic index of AM in CWP patients was higher than that in the observation subjects (P <0.05), and in the second stage patients was higher than that in the first stage patients (P <0.05) The expression levels of DR5, Caspase-3 and Caspase-8 in patients with CWP were lower than those in CWP patients (P <0.05). The expression levels of DR5, Caspase-3 and Caspase- P <0.05). The expression levels of DR5 and Caspase-3 in the 10-year-old patients were significantly higher than those in the ≥10-year group (P <0.05). Conclusion DR5, Caspase-8 and Caspase-3 may play an important role in the development of CWP. The expression of apoptosis protein is related to the length of service and the length of service.
