目的探讨妊娠期糖尿病(GDM)孕妇外周血中血红素氧合酶-1(HO-1)、脂代谢水平变化及临床意义。方法选取2014年12月-2016年10月该院收治的133例GDM孕妇为GDM组;同期随机选取30例健康孕妇为对照组。采用酶联免疫吸附法(ELISA)测定两组外周血HO-1水平,同时测定空腹血糖(FPG)、糖化血红蛋白(HbAlc)及脂代谢生化指标:总胆固醇(TC)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、甘油三酯(TG)、载脂蛋白A(ApoA)、载脂蛋白B(ApoB)。结果 GDM组外周血中HO-1水平高于对照组,血清FPG、HbAlc水平高于对照组,差异有统计学意义(P<0.05)。GDM组孕妇脂代谢生化指标HDL、ApoA水平低于对照组,LDL、TG、ApoB水平高于对照组,差异有统计学意义(P<0.05),两组孕妇TC水平差异无统计学意义(P>0.05)。经Pearson积矩相关分析,GDM组孕妇外周血HO-1水平与FPG、HbAlc、LDL、TG呈正相关(r=0.533、0.472、0.614、0.758,P=0.006、0.021、0.001、0.001);HO-1水平与HDL呈负相关(r=-0.651,P=0.002)。结论 HO-1与GDM的发生发展密切相关,脂代谢在GDM孕妇中存在不同程度的异常,HO-1参与了GDM孕妇脂代谢的调节。
Objective To investigate the changes of heme oxygenase-1 (HO-1) and lipid metabolism in peripheral blood of pregnant women with gestational diabetes mellitus (GDM) and its clinical significance. Methods A total of 133 pregnant women with GDM admitted to our hospital from December 2014 to October 2016 were selected as GDM group. Thirty healthy pregnant women were randomly selected as the control group during the same period. The level of HO-1 in peripheral blood was measured by enzyme-linked immunosorbent assay (ELISA). The levels of fasting blood glucose (FPG), HbAlc and biochemical parameters of lipid metabolism were measured: total cholesterol (TC), high density lipoprotein ), LDL, TG, ApoA, ApoB. Results The level of HO-1 in peripheral blood of GDM group was higher than that of control group, and the levels of FPG and HbAlc in serum were higher than those in control group (P <0.05). The levels of HDL and ApoA in pregnant women with GDM group were lower than those in control group, and the levels of LDL, TG and ApoB were higher than those in control group (P <0.05). There was no significant difference in TC level between the two groups (P > 0.05). The Pearson product moment correlation analysis showed that there was a positive correlation between HO-1 level and FPG, HbAlc, LDL and TG in pregnant women with GDM (r = 0.533,0.472,0.614,0.758, P = 0.006,0.021,0.001,0.001) 1 level was negatively correlated with HDL (r = -0.651, P = 0.002). Conclusions HO-1 is closely related to the occurrence and development of GDM. There are different degrees of abnormalities in lipid metabolism in GDM pregnant women. HO-1 is involved in the regulation of lipid metabolism in pregnant women with GDM.
