早在1915年,英国心电学大师Lewis就提出本位曲折的概念,即把心电图的探察电极放在心外膜某部位时,其下方心室壁的除极从心内膜开始而朝向心外膜,形成向上的除极电位。而单极胸前导联的探察电极放在胸壁,并非直接与心外膜接触,属于半直接导联或称为类本位曲折。类本位曲折的间期又称为室壁激动时间(VAT),V5导联的VAT值反映了左室心肌跨室壁完全除极的时间。正常时男性<50ms,女性<45ms。
