Aitken等1984年曾报告开放神经管畸形(NTD)、宫内死胎(IUFD)及多胎妊娠时胎儿蛋白增加向羊水内漏入或弥散。正常情况下,NTD及IUFD且伴有AChE阳性而多胎妊娠则无。作者报告一例前所未见的双胎之一胎死宫内后,健康胎儿羊水中AFP持续升高及AChE阳性。病人23岁,G_2P_0于1984年8月15日即末次月经后第17周产母血清AFP(MSAFP)浓度为210 ng/ml,相当于中值的5.4倍,一周后增至380ng/ml,为中值的8.6倍,一天后第一次超声波检查为单胎臀位,估计孕龄18.5周,遂即抽取羊水20ml清亮,AFP(AFAFP)浓度在均值以上32个标准差,AChE带显示弱阳性。8天后超声检查为双胎,一胎儿根据顶臀长度估计孕12周,位于一个单独小胎囊内,被挤在右宫底,无心跳。另一胎儿相当于孕19.5周,发育正常。孕23周时仅能取出健康胎儿之羊
