目的:研究脂联素与妊娠期糖尿病孕妇妊娠结局的关系。方法:选择妊娠期糖尿病患者(观察组)和正常健康孕妇(对照组)为研究对象,比较两组总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、脂蛋白a〔LP(a)〕、空腹血糖(FPG)、脂联素、孕产妇并发症和新生儿并发症的差异。结果:观察组妊娠期糖尿病孕妇TC、TG、LP(a)、LDL和FPG均显著高于对照组(P<0.05),HDL和脂联素均显著低于对照组(P<0.05)。观察组妊娠期糖尿病孕妇早产、羊水异常、胎膜早破和产后出血并发症发生率均显著高于对照组(P<0.05),而产后感染发生率差异无统计学意义(P>0.05)。观察组妊娠期糖尿病孕妇产新生儿体重异常、胎儿窘迫、低血糖和高胆红素血症发生率均显著高于对照组(P<0.05),而死亡发生率差异无统计学意义(P>0.05)。结论:妊娠期糖尿病孕妇脂联素水平显著低于正常健康孕妇,低脂联素血症与孕产妇和新生儿并发症密切相关。
Objective: To study the relationship between adiponectin and pregnancy outcome in pregnant women with gestational diabetes mellitus. Methods: The gestational diabetes mellitus (observation group) and normal healthy pregnant women (control group) were selected as research objects. The levels of total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL), high density lipoprotein HDL, LP (a)〕, fasting blood glucose (FPG), adiponectin, maternal complications and neonatal complications. Results: The levels of TC, TG, LP (a), LDL and FPG in pregnant women with gestational diabetes in observation group were significantly higher than those in control group (P <0.05). HDL and adiponectin in observation group were significantly lower than those in control group (P <0.05). The incidence of preterm birth, amniotic fluid abnormality, premature rupture of membranes and postpartum hemorrhage in pregnant women with gestational diabetes mellitus in observation group were significantly higher than those in control group (P <0.05), while the incidence of postpartum infection was not statistically different (P> 0.05). The incidence of neonatal weight loss, fetal distress, hypoglycemia and hyperbilirubinemia in pregnant women with gestational diabetes in observation group were significantly higher than those in control group (P <0.05), but there was no significant difference in the incidence of death (P> 0.05). Conclusion: The level of adiponectin in pregnant women with gestational diabetes is significantly lower than that of normal healthy pregnant women. Hypoadiponectinemia is closely related to the complications of pregnant women and newborns.
