目的:通过分析甘肃省社会卫生支出状况及其变动趋势,揭示甘肃省社会卫生筹资现状,为提高社会卫生筹资能力提供依据。方法:基于省级卫生总费用来源法核算社会卫生支出,计算增长速度,分析变动趋势。结果:连续20年甘肃省社会卫生支出呈现“W”型筹资特点。1995—2002呈现明显下降趋势,2002年占卫生总费用比重最低仅为18.87%;2003—2005年先升后降。2006年呈缓慢的上升趋势,2014年占卫生总费用比重为28.82%;社会卫生支出构成中社会基本医疗保险和社会办医支出为主,商业保险比重偏低,2014年占社会卫生筹资比重仅9.54%。2014年甘肃省社会卫生支出占卫生总费用比重低于全国平均约10个百分点,人均社会卫生筹资仅为633.69元,低于全国平均水平350元。结论:甘肃省社会卫生筹资能力增长缓慢,筹资能力严重不足,筹资模式和筹资渠道亟待完善优化。
Objective: To analyze the status of social health expenditure and its changing trend in Gansu Province, and to reveal the status quo of social health financing in Gansu Province, and to provide the basis for raising the social health financing ability. Methods: The social health expenditure was calculated based on the provincial health total cost source method, the growth rate was calculated, and the change trend was analyzed. Results: For 20 consecutive years, social health expenditure in Gansu Province showed “W ” type financing characteristics. From 1995 to 2002, there was a clear downward trend. In 2002, the lowest proportion of total health expenditure was only 18.87%; in 2003-2005, it increased first and then decreased. The proportion of total health expenditure was 28.82% in 2014; the expenditure on social basic medical insurance and social medicine was the main component of social health expenditure, and the proportion of commercial insurance was low. In 2014, the proportion of social health financing was only 9.54%. In 2014, the proportion of social health expenditure in total health expenditure in Gansu province was about 10 percentage points lower than the national average, and the per capita social health fund-raising was only 633.69 yuan, lower than the national average of 350 yuan. Conclusion: The financing ability of social health in Gansu Province has been growing slowly and its financing capability is seriously inadequate. The financing mode and financing channels need to be improved and optimized urgently.
