目的:探讨涎腺腺样囊性癌筛孔型中透明蛋白圆柱体的形成机理及组成成分。方法:采用了鞣酸电镜组织化学技术观察3例筛孔型腺样囊性癌。结果:在腺样囊性癌大小不等的筛孔周边分布着带状或三角形的瘤细胞,细胞胞突细长,相互吻合。胞浆中含有中间丝和肌微丝。在胞浆和胞突内可见两种类型的高尔基复合体来源的分泌囊泡。一种着色深黑,电子密度均匀,内含致密的前弹性蛋白分子;另一种着色浅灰,电子密度不均匀,内含微丝状结构的前胶原蛋白分子。细胞通过胞吐作用将这些物质释放到囊性腔隙中,聚合成胶原原纤维和弹性纤维,形成筛孔结构中的透明蛋白圆柱体。结论:腺样囊性癌筛孔型中透明蛋白圆柱体的主要组成成分是胶原原纤维和弹性纤维;透明蛋白圆柱体是由肿瘤性肌上皮细胞分泌形成的。
Objective: To investigate the formation mechanism and composition of the transparent protein cylinder in the salivary adenoid cystic carcinoma. Methods: Tannin microscopy was used to observe 3 cases of adenoid cystic carcinoma of the mesh. Results: Band-shaped or triangular tumor cells were distributed around the mesial cystadenocarcinoma of different sizes. The cytoplasm were elongated and matched with each other. The cytoplasm contains intermediate filaments and muscle microfilaments. Two types of Golgi complex-derived secretory vesicles are visible in the cytoplasm and in the cytoplasm. A dark colored, electron density uniform, dense densified pre-elastic protein molecules; another colored light gray, uneven electron density, containing microfilament-like procollagen molecules. Cells release these substances into cystic cavities by exocytosis, polymerize into collagen fibrils and elastic fibers, forming a clear protein cylinder in the mesh structure. Conclusion: The main component of the transparent protein cylinder in mesh type of adenoid cystic carcinoma is collagen fibrils and elastic fibers. The transparent protein cylinder is secreted by tumor myoepithelial cells.
