目的探讨液基薄层细胞检测技术(thin-cytologic test,TCT)和高危型人乳头瘤病毒(human papilloma virus,HPV)-DNA联合检测在宫颈癌及癌前病变筛查中的应用价值。方法 2013年2~10月在武汉市第五医院采用HPV-DNA和TCT检测的妇科检查者653名,对TCT阳性、HPV-DNA阳性病例进行阴道镜下病理组织活检。结果 HPV-DNA单一检测的灵敏度最高,但特异性、阳性预测值及诊断符合率最低;TCT单一检测的灵敏度及阴性预测值最低;TCT联合HPV-DNA检测的诊断符合率、阳性预测值及特异性均明显高于TCT、HPV-DNA单一检测(P<0.05)。结论 TCT和HPV-DNA联合检测可提高宫颈癌及癌前病变的筛查及早期诊断检出率。
Objective To investigate the value of combined detection of thin-cytologic test (TCT) and high-risk human papilloma virus (HPV) -DNA in the screening of cervical cancer and precancerous lesions. Methods From February to October 2013, 653 gynecological examiners using HPV-DNA and TCT were detected in the fifth hospital of Wuhan City. Colposcopy biopsy was performed on TCT positive and HPV-DNA positive cases. Results The sensitivity of HPV-DNA single detection was the highest, but the specificity, positive predictive value and diagnostic coincidence rate were the lowest. The sensitivity and negative predictive value of TCT single detection were the lowest. The diagnostic coincidence rate, positive predictive value and specificity of TCT combined with HPV-DNA test Sex were significantly higher than TCT, HPV-DNA single test (P <0.05). Conclusion Combined detection of TCT and HPV-DNA can improve the screening and early diagnosis of cervical cancer and precancerous lesions.
