目的 探讨海马硬化与颞叶癫 (TLE)关系及在TLE外科治疗中的作用。方法 对 15例TLE并考虑海马硬化的病例实施选择性海马杏仁核切除。结果 全部病例术中在皮质脑电图 (EEG)及深部EEG均发现颞叶内侧样放电 ,手术切除硬化海马后癫得到有效控制 ,术后患者神经心理正常。结论 海马硬化可能是TLE的主要病因 ,核磁共振 (MR)发现海马硬化是TLE定位诊断的重要依据 ,手术切除硬化海马可对TLE起到治疗作用。
Objective To investigate the relationship between hippocampal sclerosis and temporal lobe epilepsy (TLE) and its role in the surgical treatment of TLE. Methods Selective hippocampal appendectomy was performed on 15 cases of TLE and considering the hippocampal sclerosis. Results All the cases were found in the medial temporal lobe of EEG and deep EEG during operation, and the epilepsy was effectively controlled after the surgical excision of the sclerosis hippocampus. The postoperative neuropsychology was normal. Conclusion Hippocampal sclerosis may be the main cause of TLE. Hippocampal sclerosis was found to be an important basis for the diagnosis of TLE by nuclear magnetic resonance (MR). Surgical resection of the hardened hippocampus may play a therapeutic role in TLE.
