目的研究气管支气管及肺动脉成形术的适应证、术中处理及并发症的防治。方法回顾性分析接受气管支气管及肺动脉成形术肺癌患者26例的临床资料。结果全组患者住院时间均少于30d,无住院死亡。术后并发肺不张3例,气胸1例,单纯脓胸1例,均经治疗后痊愈出院。23例获随访12～78个月,2例于术后1年死亡,3例于术后3年死亡。结论气管支气管及肺血管成形术治疗中央型肺癌不仅能最大限度地保留肺组织,提高患者术后生存质量,且为部分肺功能差的患者提供了切除病变的机会。
Objective To study the indications of tracheobronchial and pulmonary angioplasty, intraoperative management and prevention and treatment of complications. Methods The clinical data of 26 patients with lung cancer receiving tracheobronchial and pulmonary angioplasty were retrospectively analyzed. Results All patients in hospital stay less than 30d, no hospital deaths. Postoperative pulmonary atelectasis in 3 cases, 1 case of pneumothorax, empyema in 1 case, were cured after treatment were discharged. Twenty-three patients were followed up for 12-78 months. Two patients died one year after operation and three patients died three years after operation. Conclusion Tracheobronchial and pulmonary angioplasty for central lung cancer can not only maximize the lung tissue retention and improve postoperative quality of life, but also provide opportunities for some patients with poor lung function to remove the disease.
