目的:分析新型农村合作医疗脑梗死患者住院费用的变化及影响因素。方法:收集2011—2014年安徽省9县新型农村合作医疗脑梗死病人费用清单,采用描述性统计和多元线性回归方法,分析住院费用的变化趋势、报销情况和影响因素。结果:脑梗死患者住院人数、次均住院费用和日均住院费用均呈上升趋势,平均住院日和次均药占比呈下降趋势;报销比例提高,自付占农村家庭人均年收入比例下降;地理分区、医疗机构类别、住院日和药占比是影响住院费用的主要因素。结论:脑梗死患者数呈上升趋势,住院费用增长速度降低;新型农村合作医疗报销比例升高,疾病经济负担有所缓解;平均住院日越长、机构级别越高,住院费用越高。
OBJECTIVE: To analyze the change of hospitalization expenses and its influencing factors in new rural cooperative medical patients with cerebral infarction. Methods: To collect the cost list of new rural cooperative medical patients with cerebral infarction in 9 counties in Anhui province during 2011-2014. The descriptive statistics and multivariate linear regression were used to analyze the changing trend, reimbursement and influencing factors of hospitalization expenses. Results: The number of hospitalizations, the average cost of hospitalization and the average daily hospitalization costs of cerebral infarction patients showed an upward trend, the average length of hospital stay and the proportion of average drug use decreased. The proportion of reimbursement increased and the proportion of self-payment to rural household per capita income decreased. Geographical zoning, types of medical institutions, days of hospitalization and the proportion of drugs are the main factors affecting hospitalization costs. Conclusions: The number of patients with cerebral infarction is on the rise, the rate of increase of hospitalization expenses is lower; the proportion of reimbursement of new-type rural cooperative medical care is increased, the economic burden of illness is eased; the longer the average hospitalization day, the higher the institution level, the higher the cost of hospitalization.
