目的 研究耳 体穴针刺防治胃肠道肿瘤术后TPN引起的肠粘膜屏障功能损害及促进肝细胞代谢的意义。方法 胃肠道肿瘤术后行TPN支持的病人 (n =40 ) ,实验组 (n =2 0 )配合耳 体穴针刺 ,两组同时观察胃肠道功能恢复时间、肝功能各项指标、术前及术后第 7天作乳果糖排泄率 /甘露醇排泄率 (L/M)实验。结果 术前后组内与组间比较 ,AST、ALT、TBIL、AKP均无增高 ,血浆白蛋白术后第三天较术前有明显下降 (P <0 .0 1) ,术后第 7天均有明显回升 ,研究组比对照组升高明显 (P <0 .0 5 )。术后第 7天 ,对照组与术前及与实验组比较 ,6h尿L/M明显增大 (P <0 .0 1) ;而实验组手术前后无显著性差异 (P >0 .0 5 )。结论 胃肠道肿瘤术后行TPN支持的病人 ,配合针刺可保护肠粘膜屏障、促进肝细胞代谢功能。
Objective To study the significance of acupuncture on the intestinal mucosal barrier dysfunction induced by TPN and the promotion of hepatic cell metabolism after acupuncture at ear cavity. Methods Twenty patients with gastrointestinal tract tumor (n = 40) were treated with TPN. The experimental group (n = 20) was acupunctured with acupoints. The recovery time of gastrointestinal function, indexes of liver function, The lactose excretion rate / mannitol excretion rate (L / M) test was performed preoperatively and on the 7th day after operation. Results The levels of AST, ALT, TBIL and AKP were not increased in the preoperative and postoperative groups. Compared with the preoperative and postoperative levels, plasma albumin significantly decreased (P <0.01), on the 7th day All significantly increased, the study group than the control group increased significantly (P <0.05). On the 7th day after operation, the urinary L / M at 6h was significantly increased in the control group compared with the preoperative and experimental groups (P <0.01), while there was no significant difference in the experimental group before and after surgery (P> 0.05) ). Conclusions Patients with gastrointestinal cancer who underwent TPN after operation may cooperate with acupuncture to protect the intestinal mucosal barrier and promote the metabolism of liver cells.
