目的探讨乳腺肿瘤CT灌注的诊断价值。方法对17例钼靶X线检查发现乳腺肿块性病变(21个病灶)行CT灌注扫描,使用电影模式,0.5s/圈,5mm层厚×4,120kV,60mA,延迟时间10s,总扫描时间50s,使用GE AW4.2工作站和Perfusion3软件包,对生成的动态图像数据和图像强度随时间变化的相关信息进行分析,考察各种与灌注相关的参数情况,并进行统计学分析。结果乳腺癌组:血流量(BF)(36.46±17.62)ml·min-1.100g-1,血容量(BV)(13.76±8.59)ml/100g,平均通过时间(MTT)(28.23±15.75)s,血管表面通透性(PS)(16.45±12.36)ml·min-1.100g-1,良性肿块组血流量(BF)(17.35±10.67)ml·min-1.100g-1。良性肿块组:血容量(BV)(4.63±3.47)ml/100g,平均通过时间(MTT)(25.52±12.91)s,血管表面通透性(PS)(3.57±3.36)ml·min-1.100g-1。2组BF、BV、PS之间的差异有统计学意义(P<0.05)。结论CT灌注成像提供了乳腺肿块的血流动力学信息,在乳腺肿块性病变的良恶性鉴别诊断中有较高的参考价值。
Objective To investigate the diagnostic value of CT perfusion in breast tumors. Methods CT scan was performed in 17 cases of mastoid lesions (21 lesions) on 17 cases of mammography. Results The breast cancer group had significantly higher BF (36.46 ± 17.62) ml · min-1.100g-1, BV (13.76 ± 8.59) ml / 100g and MTT (28.23 ± 15.75) s , Vascular surface permeability (PS) (16.45 ± 12.36) ml · min-1.100g-1 and BF (17.35 ± 10.67) ml · min-1.100g-1 in benign tumor. The volume of BV was (4.63 ± 3.47) ml / 100g, the mean transit time (MTT) was 25.52 ± 12.91 s, the PS was 3.57 ± 3.36 ml · min-1.100g The difference between -1.2 group BF, BV, PS was statistically significant (P <0.05). Conclusion CT perfusion imaging provides the hemodynamic information of breast masses and has high reference value in the differential diagnosis of benign and malignant lesions of breast lesions.
