目的探讨踝关节粉碎性骨折围手术期采用无痛理念配合温馨护理对应激状态的影响。方法选取踝关节粉碎性骨折患者70例,随机分为对照组与研究组各35例,对照组给予常规护理,研究组在对照组的基础上给予无痛理念配合温馨护理,观察两组患者围手术期的疼痛和应激激素水平的变化。结果两组术后6 h及24 h的疼痛评分均明显小于术前,术后24 h评分均明显小于术后6 h,且研究组术后疼痛评分显著小于对照组,差异均有统计学意义(P<0.05)。两组术后1 d、术后3 d的Cor、ALD、AngⅡ水平均明显高于术前,且术后3 d明显高于术后1 d,但研究组明显低于对照组,差异均有统计学意义(P<0.05)。结论无痛理念配合温馨护理能够有效减轻踝关节粉碎性骨折患者围手术期的疼痛感,并能够有效缓解其血清应激激素的水平表达。
Objective To investigate the effect of applying painless concept combined with warm nursing on stress state in perioperative period of comminuted fracture of ankle joint. Methods Seventy patients with ankle comminuted fracture were randomly divided into control group and study group with 35 cases each. The control group was given routine nursing. The study group was given painless concept and warm nursing on the basis of the control group, Changes in operative pain and stress hormone levels. Results The scores of pain at 6 h and 24 h after operation in both groups were significantly lower than those before operation and at 24 h after operation, respectively, which were significantly lower than those at 6 h after operation, and the postoperative pain scores of the study group were significantly lower than those of the control group (P <0.05). The levels of Cor, ALD and AngⅡ on the first day and the third day after operation in both groups were significantly higher than those before operation and significantly higher than those on the first day after operation, respectively, but were significantly lower in the study group than in the control group Statistical significance (P <0.05). Conclusions The painless concept combined with warm nursing can effectively reduce perioperative pain in patients with ankle comminuted fracture and can effectively relieve the level of serum stress hormone.
