目的:探讨柴芩承气汤(CQCQD)对急性坏死性胰腺炎大鼠胰腺组织抗生素浓度的影响。方法:18只SD大鼠随机分成三组(正常组、ANP组、CQCQD组),每组6只。ANP造模采用3.5%牛磺胆酸钠(1ml/kg)逆胰胆管注射。造模成功后6h,大鼠腹腔注射CTX(420mg/kg,Q12h,连续72h);CQCQD组同时管喂中药(20ml/kg,Q8h,连续72h)。于第72h最后一次注射CTX 15min后取血样和胰腺组织。用高效液相色谱法测量血清及胰腺组织的CTX浓度。结果:大鼠血清CTX浓度在三组间无差异。与正常组比较,ANP组大鼠胰腺组织的CTX浓度由前者的18.6±1.7(μg/ml)降至4.4±0.6(μg/ml),穿透率由19%降到5%(P<0.01),两者比较有显著的统计学差异。中药CQCQD干预后,大鼠胰腺组织的病理评分示炎症和水肿改变较ANP组减轻,干湿重比值下降(P<0.001);CQCQD组大鼠胰腺组织的CTX浓度升高到6.4±1.7(μg/ml),穿透率升至7%(P<0.05),与ANP组比较有统计学差异。结论:中药CQC-QD能增加大鼠CTX的胰腺穿透率和局部组织药物浓度,其具体机制尚待进一步研究。
Objective: To investigate the effect of CQCQD on antibiotic concentration in pancreatic tissue of rats with acute necrotizing pancreatitis. Methods: Eighteen SD rats were randomly divided into three groups (normal group, ANP group, and CQCQD group) with 6 rats in each group. ANP model using 3.5% sodium taurocholate (1ml/kg) reverse pancreatic bile duct injection. Six hours after the model was established, rats were injected intraperitoneally with CTX (420mg/kg, Q12h for 72 hours), and CQCQD group was given simultaneous Chinese medicine (20ml/kg, Q8h, 72h continuous). Blood samples and pancreatic tissue were taken after the last injection of CTX at 72h. Serum and pancreatic tissue CTX concentrations were measured by high performance liquid chromatography. Results: There was no difference in serum CTX concentration between the three groups. Compared with the normal group, the CTX concentration of pancreatic tissue in the ANP group decreased from 18.6±1.7 (μg/ml) to 4.4±0.6 (μg/ml), and the penetration rate decreased from 19% to 5% (P<0.01). ), there are significant statistical differences between the two. After intervention of Chinese medicine CQCQD, the pathological scores of pancreatic tissue in rats showed that inflammation and edema changes were less than those in ANP group, and the ratio of dry to wet weight was decreased (P<0.001); the CTX concentration in pancreatic tissue of rats in CQCQD group was increased to 6.4±1.7 (μg /ml), the penetration rate rose to 7% (P<0.05), and there was a statistically significant difference from the ANP group. Conclusion: The traditional Chinese medicine CQC-QD can increase the pancreatic penetration rate and local tissue drug concentration of rat CTX, and its specific mechanism needs further study.
