目的:评价结核感染T细胞斑点试验(T-SPOT.TB)诊断儿童结核病的应用价值。方法:选择2010年11月至2012年2月昆明市第三人民医院住院的患儿为研究对象。对入院未治疗的32例临床诊断病例的结核病患儿,31例非结核病患儿分别进行T-SPOT.TB试验、结核菌素试验(PPD皮试)、血沉(ESR)、结核抗体(TB-DOT/TB-CK)检测;比较其敏感性和特异性。结果:T-SPOT.TB试验诊断儿童结核病的敏感性为90.6%,特异性为96.8%;PPD试验敏感性为83.3%,特异性为71.4%;TB-DOT/TB-CK敏感性为36.7%,特异性为90.3%;ESR敏感性为63.0%,特异性为26.7%。结论:T-SPOT.TB诊断小儿结核病敏感、快速,而且受其它干扰因素的影响较小,对儿童结核病具有较高的诊断价值,可用于儿童结核病的早期快速诊断。
Objective: To evaluate the value of T-SPOT.TB in the diagnosis of tuberculosis in children. Methods: The children hospitalized in the Third People’s Hospital of Kunming from November 2010 to February 2012 were selected as the study subjects. T-SPOT.TB test, tuberculin test (PPD skin test), erythrocyte sedimentation rate (ESR) and tuberculosis antibody (TB-TBT) were performed on 32 untreated patients with tuberculosis and 31 non- DOT / TB-CK) test; compare the sensitivity and specificity. Results: The sensitivity and specificity of T-SPOT.TB test for diagnosis of childhood tuberculosis were 90.6% and 96.8% respectively. The sensitivity and specificity of PPD test were 83.3% and 71.4%, respectively. The sensitivity of TB-DOT / TB-CK was 36.7% , Specificity was 90.3%; ESR sensitivity was 63.0% and specificity was 26.7%. Conclusion: T-SPOT.TB diagnosis of tuberculosis in children is sensitive and rapid, but also less affected by other interference factors, has a high diagnostic value in children with tuberculosis can be used for rapid early diagnosis of tuberculosis in children.
