目的:探讨激素环境下圆窗进路人工耳蜗植入术后不同时期电极阻抗值的变化,为术后言语处理器的调试提供依据。方法:对2014-09-2015-10期间在地塞米松环境下行人工耳蜗电极植入(Model:sonata)的47例患者进行术后电极阻抗动态变化跟踪检测,将不同时期的阻抗值进行对比,另根据频率不同进行分组,对比不同频段组间的阻抗值,进行统计分析。结果:全频段及3个亚组的平均阻抗在术中(A0期)最小,术后1个月(A1期)达到最大值,随后缓慢下降,6个月(A3期)开始趋向稳定;6个月(A3期)与1年(A4期)的数值比较差异无统计学意义,其他各时期之间均差异有统计学意义(P<0.05);5个时期中,均是低频段的平均阻抗值最高,中频段的平均阻抗值最低。结论:局部应用激素对人工耳蜗植入术后降低电极阻抗具有一定作用。激素环境下圆窗入路人工耳蜗植入术电极阻抗值在术中最小,术后1个月达最大值,随后缓慢下降,术后6个月时趋向稳定;因此开机到术后6个月期间应定期调试机器,以获得最佳听力状态。
OBJECTIVE: To investigate the changes of electrode impedance at different periods after cochlear implantation under the round window approach in hormonal environment, and to provide a basis for postoperative speech processor debugging. Methods: Forty-seven patients undergoing cochlear implant implantation (Model Sonata) under the dexamethasone environment during the period of September 9, 2014 to October 15 were followed up with the dynamic changes of the electrode impedance. The impedance values at different periods were compared. The other according to the frequency of different groups, compared with different band impedance values between groups for statistical analysis. Results: The average impedance in the whole frequency band and the three subgroups was the lowest during the operation (A0 period), reaching the maximum value at 1 month (A1 period) and then slowly decreasing, and stabilized at 6 months (A3 period). 6 There was no significant difference between the three months (A3) and the one year (A4), but there was a significant difference among the other periods (P <0.05) The highest impedance, the lowest average impedance in the middle band. CONCLUSIONS: Topical application of hormones has a role in reducing electrode impedance after cochlear implantation. Under the hormonal environment, the electrode impedance value of cochlear implants at the round window approach was the lowest during operation, reaching the maximum at 1 month after operation and then slowly decreasing. After 6 months, the impedance tended to be stable; During the regular debugging of the machine, in order to obtain the best hearing status.
