目的探讨高血压病(EH)患者24h平均脉压(APP)与左室肥厚(LVH)的关系。方法对90例高血压病患者进行24h动态血压监测及心脏超声检查,根据有无LVH分为两组:LVH组、非LVH组,再以PP≥60mm Hg、PP<60mm Hg分为2个亚组。结果 LVH组与非LVH组24h APP分别为(76.3±16.5),(56.3±10.6)mm Hg、收缩压分别为(156.4±7.26),(146.7±11.0)mm Hg,差异均有统计学意义(P<0.05),而两组舒张压分别为(92.5±5.17),(90.2±6.25)mmHg,之间差异无统计学意义(P<0.05),PP≥60mmHg组LVH发生率较PP<60mmHg组显著增加(P<0.01)。结论高血压患者24h APP与LVH密切相关,脉压增大是心血管病的危险因素。
Objective To investigate the relationship between 24h mean arterial pressure (APP) and left ventricular hypertrophy (LVH) in patients with essential hypertension (EH). Methods Ninety hypertensive patients underwent ambulatory blood pressure monitoring and echocardiography at 24 hours. According to the presence or absence of LVH, they were divided into two groups: LVH group and non-LVH group, and divided into two subgroups: PP≥60mm Hg and PP <60mm Hg group. Results The 24h APPs were (76.3 ± 16.5) and (56.3 ± 10.6) mm Hg in LVH group and non-LVH group respectively, and the systolic blood pressures were (156.4 ± 7.26) and (146.7 ± 11.0) mm Hg, respectively (P <0.05), while the diastolic blood pressure of the two groups were (92.5 ± 5.17) and (90.2 ± 6.25) mmHg respectively, with no significant difference (P <0.05). The incidence of LVH in PP≥60mmHg group was significantly higher than that of PP <60mmHg group Significantly increased (P <0.01). Conclusion 24h APP in patients with hypertension is closely related to LVH. Increased pulse pressure is a risk factor for cardiovascular disease.
