目的:检测α-平滑肌肌动蛋白(α-SMA)、基质金属蛋白酶-2(MMP-2)、碱性成纤维生长因子(b FGF)在正常子宫颈、宫颈高级别上皮内瘤变(CINIII)、宫颈早期浸润癌(MIC)间质中的表达,探讨其在CINIII和MIC鉴别诊断中的价值。方法:收集110例全子宫切除或宫颈锥切的标本,其中正常宫颈组织30例、CINIII组50例、MIC组30例。免疫组化法检测3组间质细胞中α-SMA、MMP-2、b FGF表达。结果:MIC组、CINIII组和对照组宫颈间质细胞中α-SMA阳性表达率分别为90.0%、70.0%和0%,MMP-2阳性表达率分别为100.0%、92.0%和40.0%,b FGF阳性表达率分别为40.0%、18.0%和3.3%;3组比较差异均有统计学意义(P<0.001)。结论:α-SMA、MMP-2及b FGF从正常宫颈、CINIII组到MIC组,其表达强度逐渐升高,可联合用于CINIII与MIC的诊断与鉴别诊断。
Objective: To detect the expression of α-smooth muscle actin (α-SMA), matrix metalloproteinase-2 (MMP-2) and basic fibroblast growth factor (b FGF) in normal cervix and cervix high grade intraepithelial neoplasia ), Early cervical cancer (MIC) interstitial expression, to explore its value in the differential diagnosis of CINIII and MIC. Methods: Totally 110 specimens of total hysterectomy or conization of the cervix were collected, including 30 cases of normal cervical tissue, 50 cases of CINIII group and 30 cases of MIC group. Immunohistochemistry was used to detect the expression of α-SMA, MMP-2 and b FGF in three groups of mesenchymal cells. Results: The positive rates of α-SMA in cervical stromal cells in MIC group, CINIII group and control group were 90.0%, 70.0% and 0%, respectively. The positive rates of MMP-2 were 100.0%, 92.0% and 40.0% The positive rates of FGF were 40.0%, 18.0% and 3.3%, respectively. There were significant differences among the three groups (P <0.001). CONCLUSION: The expression of α-SMA, MMP-2 and b FGF in normal cervix and CINIII group increases to the MIC level, which can be used in the diagnosis and differential diagnosis between CINIII and MIC.
