目的探讨应用吲哚菁绿联合红外成像的方法诊断肢体获得性淋巴水肿,并在术前进行淋巴管定位的可能性及准确性。方法于患肢及对侧正常肢体第2指或第2趾间隙注射吲哚菁绿,注射后即刻、30 min、2 h分别行患肢及对侧正常肢体红外成像,术前根据吲哚菁绿造影进行淋巴管定位,在手术中对其准确性进行检测。结果共18例肢体获得性淋巴水肿患者进行淋巴管成像,吲哚菁绿红外成像后标记的淋巴管与术中探查淋巴管位置一致率为95.24%,假阳性率为4.76%。结论吲哚菁绿显像是一种安全、微创的造影方法,可以对核素造影获得的信息进行有效的补充,该方法对于淋巴管的定位及通畅度检测可获得准确结果,对淋巴水肿手术方法的选择具有指导意义。
Objective To investigate the feasibility and accuracy of using indocyanine green combined with infrared imaging to diagnose limb lymphedema and to locate lymphatic vessels preoperatively. Methods Indocyanine green was injected into the second or second interphalangeal space of the affected limbs and the contralateral normal limbs. Infrared imaging of the limbs and the contralateral normal limbs were performed immediately, 30 min and 2 h after injection. According to indocyanine Green angiography for lymphatic mapping, its accuracy in the operation of the test. Results A total of 18 patients with limb-acquired lymphedema underwent lymphatic imaging. The coincidence rate of lymphatic vessels labeled with indocyanine green-infrared imaging and lymphatic vessels detected during operation was 95.24% and the false-positive rate was 4.76%. Conclusions Indocyanine green imaging is a safe and minimally invasive radiography method, which can effectively supplement the information obtained by radiography. This method can accurately detect the lymphatic vessel’s location and patency, The choice of surgical method is instructive.
